Going up?

I've always thought that elevators were an awkward place to be..you step into a small room with other people you dont know and basically stand around doing nothing..its just awkward..
they did a lot of experiments in the 50's about elevator psychology..anywhere in the world, is the same story..
next time you're in an elevator, try and look for the telltale signs of awkwardness..
some people look at the floor
some people look at their watches
others will fidget
and the number 1 sign of awkwardness is .. drum roll please..its looking up to check at that numbered display..
most elevators nowadays actually announce which floor youre at but people still stare at that number
they do that for a reason..
they're quietly thinking to them self wishing for the ordeal to be over with...
dont believe me?
next time youre in the elevator, go to the back and look at the other people..
everyone will deal with it in their own way..
plus, by doing this, you will gain a sudden false sense of superiority and lose your own awkward feeling..

the s*it i do keep from feeling useless in an elevator..

I hate fish..

I hate fish...especially if theres someone stupid enough to eat the fish and get a fishbone stuck in their throat..these people should only be licensed to eat fillet-o-fish or fish and chips..
here's a few tips if you're contemplating on eating fish

1. before you eat the fish, think really hard..you wouldn't want to see me at the emergency department when you accidentally (or rather stupidly) swallowed the damm bone..
2. just remember this, chicken tastes wayyy better..
3. if you have chosen to ignore advice number 1 and 2, please shut up and concentrate when you're eating..dont talk, laugh or sneeze..look at what you are putting in you're mouth..
4. please, please, please check to see that theres no bone before you swallow..the easiest method to do this is to chew your food..yes..its that simple..chew
5. if you have loose dentures, go get it bloody fixed.. you're in enough sh*t in the first place if you have to wear them.. dont complicate your life further by eating fish..
6. always refer back to rule number 1..

and to the doctors, i'm that tired of repeating myself that i have decided to put this in cyberland..if someone has swallowed a fish bone (stupidly), please do the following:

1. ask them when
2. what fish. (small or big boned)
3. was it fried fish? (cause fried ones are friable)
4. ask if the pain is sharp and pricking on every swallowing of saliva. if it is, the bone is still there.
5. wheres the pain.
      - is it on the left or right side and which level is it.
      - by level, i mean hyoid, thyroid or cricoid level. give those references.. vertebral levels are also ok.
      - saying 'somewhere in the middle of the neck' does not sound to intelligent'
      - if the pain is above the thyroid cartilage, the stupid bone is usually at the tonsils. which leads me to..
6. Look at the oral cavity..really look for the bone at the tonsils and base of tongue..most of the time its there..
7. Look agaiinnn..
6. Do a soft tissue neck x-ray..not cervical x-ray..not AP view..but lateral neck soft tissue..
7. This one is a dont..dont say 'i cant see the fish bone in the xray' as your opening line..usually it cant be seen..so i wouldn't really care if you didn't..what you should look for is:
       - loss of cervical lordosis - means pt is in pain
       - increase of soft tissue at the bit behind the trachea up to the vertebra..any widening shows
          inflammation. its significant if the distance of soft tissue is more then the width of the vertebra.
       - any air trapping in the esophagus. if there is air trapping, thats where the foreign body is..
       - after all that, then you can mention that you cant see the stupid bone that the patient swallowed

i do understand that getting a fishbone stuck is sometimes unavoidable...
on second thought, unless you got some swallowing problem, it is avoidable..
but first time offenders are usually ok..human error..
Its the repeat offenders that tick me off..

Points..

You'll know that someone has been a government doctor too long when the reason they suggest to you to join a music class is to gain cpd (continuous professional development) points or a higher skt point (sasaran kerja tahunan)..
i was in Bentley (the music store, not the car shop) today..just browsing thru their instruments..missed the days where i can just drop everything, call a few friends, book a studio on the spot and then indulge in a jamming session..
those were the days..
now my hand feels awkward when holding the neck of a guitar..not to mention that garbage instead of music comes out when i try to play it..yes, i did not exaggerate.. garbage..
on the up side, i still knew how to hold the drumstick the right way around..
but that's as far as it stretches..i managed to make a 45 thousand dollar state-of-the-art electronic drum set sound like, wait for it ...garbage..pure and utter garbage again..
i'd take it as a compliment if 'garbage music' is currently trendy and is in fact 'the thing' that people listen to nowadays..but you can be safe in knowing that it only happened in my own world that i had to retreat to once i started playing..
coincidentally, the people in that world really digs frame-less spectacles..i wonder why..

aaaanywys, guess i'd better sign up for that music class..not to play better..hell no..only for the cpd points..

Hari Raya...

So..its been a while..
sorry bout that..been occupied..
had a good 2 day break after the exams before diving straight into a 2 day weekend call..
no complaints though..it was ok..
anyways, the main reason for the post is to say selamat hari raya for all my friends and family who actually reads my ramblings over the past few months..

Memory..

Everyone whose working will say that studying is better then working..
its true until 'they' start studying again..
then 'they' just wish that 'they' were 'working' again..
and by 'they', i mean myself..
and by 'working', i mean sitting by the beach sipping coconut juice..nice..
i hate the memorizing part of studying..
i've always had a photographic memory..
the problem is that i can never find the film..
pisses me off..

Between calls and patients...

i just had to go to the emergency department to see an 18yr old with a right peritonsillar abscess
the good thing was that these are the very few times that the emergency MO got the diagnosis right..
so i wasn't pissed off at all when i got there..
that is until i met the patient..
she was unwilling to speak, eat or open her mouth..
she kept the abscess for the past 1 week..
seriously ticked me off..
baik balik rumah and keep it another week je if camtuh..
i see that with a lot of patients..waiting until the last min before they actually come to hospital..
really Malaysian culture..
when will we change..

p/s: thank god i've got a place to vent..this 'service with a smile' thing takes a real big chunk out of you if you have to deal with people's stupidity..

Dont people work on Mondays anymore?

This is the story of the very few times i get to not show up for work on a monday..
i decided to get a haircut and get some groceries on my day off only to find that, its full..
evereywhere is full of people..
it looks like a weekend..but its a monday..
dammit..
there goes the sanctity of a monday leave from work..

on another more relaxed note, my friend showed me klpac district the other day..
its unimaginable that that place is in the middle of kl..
it was a nice change..
if only i can get another monday off so i can take my wife there..hmmm...

Damm...that was a cool movie..confused the hell out of me though..the movie actually used basic concepts that were  delirious..it swayed between  reality and dreams so much that you are left dazed and lost in the transitions..unfathomable circumstances and distorted realism was a major theme..on top of that, i was pissed at the ending ..just tell us whether it was real or not man..your brain should hurt bout 3/4 of the way thru the movie..why did they have to make us think more with an ending like that..
but like i said,  it was cool none the less..

Good morning...

Hello again folks..it's been a while..
that is of course with purpose..
i wanted to see whether the counter thingy kept moving despite having no posts..
400..thats how much it went up..
and now that i know that , i can happily re-enter the blogverse..

Anyways, i'm up early today..
amazing right? you might think there is a reason why i woke up early on a saturday..
there's actually a lot of reasons but the most pertinent one is the most depressing..
do you remember in form 3 (or 5..cant remember..) where they thought you about Pavlov and that dog..
the one where the dog (or was it in med school? dammit..) salivates when the bell rings?
i am that dog..
i find nowadays, i wake up in the morning when i sense morning sunlight
i'm just awake..
but bloody tired..(i 've been thinking bout this too..maybe in another post..)
thats pathetic right?
sometimes it's too simple to condition the human mind..
look at hand phones for example..how many times have you checked the time on your phone when you could have checked it on your watch..
its too simple i tell you..too effing simple..

so the conclusion is, i'm pissed that i woke up too early today..

What have i gotten myself into..

Folks, i did it..i finally paid for the exam..it took me long enough to commit to it..and now i'm 2000 bucks short with an exam due on the 1st of September..thats 80 days from now..

what was i thinking..

now if only i can subscribe to luck and ensure i get a continuous flow of it till exams..

anyways, went to see prince of persia yesterday with my wife and a few friends..
suffice to say that they didn't destroy the storyline..
maybe i didn't set my standards high enough this time around..did that with iron man 2 and that turned out to be quite a disappointment..even Shrek was more enjoyable then that one..
Anyways, i'm on call again today..lets hope for the best..

Cant Afford a Lomo..

have been laying low from blogland for a while..decided to put up some pics instead..

i really like how lomo perks up colours..gives nice highlights..makes everything looks a bit retro and surreal..anyways.. these are actually dslr images redone in lomo style with photoshop..cant say they are 100% accurate or anything but what i wanted was for the colours to show up..

Superstitious..

Are you superstitious? if you say you aren't, you're probably lying..really..
have you ever avoided cracks on the roads? ever felt a bit freaked out by a totally black cat? ooo..and the mirror thing.. thats one of the big ones..if any of these struck a bell, then you are superstitious..you might be saying that 'i just avoid those things some of the times..thats ok right?"
he*l no..that just means that you're in bloody denial, (and are a borderline liar) but you are superstitious anyway..
but don't worry..cause i am too..it started the moment i started my first on-call as a doctor..
oh yes..you are reading it right..during my first on call..you see, i think that doctors are one of the most superstitious group out there..the believe it the power of 'jona'..
whats jona you ask? jona is the curse some people inheritly have to attract patients during their on call..
in most cases, this power can be stronger than that force crap they talk about in star wars..
as you might have guessed by now, i am jona too..
i somehow attract patients like moth to a burning flame..seriously..
and the sh*t i see during my on calls are just not funny..messed up cases..even my bosses will agree with that..
since that first on-call and over the years of working, i have devised a few things that i think (keyword is think people) might dampen my jona-ness..one of them is not to wear dark colours during on calls..that includes your underwear..(some wise-ass is sure to ask that)..and you gotta think happy thoughts and don't let yourself feel agitated..some doctors believe taking a shower before they start their call..i tried that and it didn't work..3 patients died that night when i actually showered so thats off my list..
so you see, i think that no one is immune to superstitions. . i know this jona thing is silly but i still do it anyway..my default defense mechanism is by saying its better being safe then sorry..regardless of your excuses, you still aresuperstitious..so just live with it..its one of the unexplainable neurosis of life..

its been good for me..nothing much happening..

Escape..

Had a nice getaway trip last Saturday..
There's something about the beach that puts your mind at ease..
just chilling and enjoying the seabreeze can really take away the accumulated scars of daily life..
There's a lot of scars man..
Most of it is due to waking up early to go to work..

What to do today?

The question that haunts us all..seems like there's a lot of productive things to settle but my mind keeps telling me to screw them all and just relax..

Sigmund Freud says the mind is composed of the conscious ant the subconscious.. the subconscious mind is further divided into the id, ego and superego..the id is the impulsive nature of a person ant the superego is something like the more sensible part of the subconscious..for example if i see a band new VW scirocco in front of me, the conversation between the three might be something like:

id: i want..
ego: should i steal it?
superego: you two dumb f**ks shut up. you cant afford it and no way in hell you're gonna steal it..probably cant even fit in the drivers seat..

the way i see it, superego has won too many battles..so today, i have decided that I'd drown my superego and let the id win..i shall just relax..just hope they don't call me for a case today...

yes folks..its that time again..
its that time when i crack my brain open trying to find that special needle of wisdom in a swirling haystack of useless nothingness to catalog into the world wide web so that i may share it to anyone who gives a damn..

Why do weekends fly by?

i woke up today thinking,
'shit..i'm oncall tomorrow'..

that was at 8am on sunday morning..
yes..
8am..
believe it..
thats the reason why sundays dont really count as the weekend..if it's not oncalls' its work..either way you cant spend your sunday in a calm and cool mood..i'd add 'being blissfully ignorant of mondays' anytime to my wish list..at least that way i'll be able to squeeze a bit more juice out of the weekends..

to me, i consider the weekends starting at 5pm friday and ending on around 9-10am sunday..the rest of the sunday is usually spent fretting about work on monday..either that or you're running around trying to tie the loose ends in preparation for the upcoming week..thats sucks donkey balls dont it? its kinda one of life's thorns thats has been genetically engineered to prick you just as you are getting comfortable..

anyways, nothing much to do bout it..the monday blues just hitting harder then usual this time around..just my two cents..

p/s: if you dont work on mondays, good for you..but i'd go bruce lee on your ass if you gloat..ahahaha..later..

neat huh? never thought i'd actually set up a blog.. I've always thought it was kinda cool but my laziness; which is more of a talent then anything else, usually gets the better of me..then this morning, as i was opening my wife's pc, i saw that she had a blog..my god..my own Miss 'can you please install an antivirus for my laptop cause i'm not sure how' has a blog..

but wait..

its not just a nonsense blog..it's got useful information..i thought to myself, i cant compete with useful..i'm spending more energy then i want to just by writing this..so as the CEO and principal shareholder of this blog i have decided on its course..i shall write about...wait for it..'stuff'..

oh just wait and see..
